#1 - Intracellular reactive oxygen species production and phagocytosis of Staphylococcus aureus by milk neutrophils as tool to diagnose mastitis and identify susceptible dairy cows

Abstract in English:

The immune response capacity of the mammary gland plays a major role to determine if mastitis will or not be established. Thus, we hypothesize that a better understanding of polymorphonuclear neutrophil leukocyte (PMN) function will elucidate mechanisms that will improve our knowledge of how we could avoid an inflammatory process by increasing the immune capacity of the cow, and even further, to search for a tool to diagnose mastitis or a possible way to select and identify non-susceptible animals. The present study utilized 112 quarters from 28 Holstein dairy cows that were divided into quarters milk samples with somatic cell count (SCC) <2×105 cells mL-1 (n=72) and SCC >2×105 cells mL-1 (n=40). The percentages of milk PMNs and the levels of intracellular reactive oxygen species (ROS) and the phagocytosis of Staphylococcus aureus by milk neutrophils were evaluated by flow cytometry. Our results showed a higher percentage of neutrophils in quarter milk samples with high SCC (P=0.0003), and this group also had a significantly higher percentage of neutrophils that produced ROS (P=0.008). On the other hand, the phagocytosis intensity of S. aureus by milk neutrophils was higher in quarters with low SCC (P=0.003), suggesting a better mammary gland immunity against invading pathogens. Analyzing the results of the predictive values of the measured PMN functions, they cannot be used isolated as a good diagnosis test since none of them had a satisfactory sensitivity and specificity values, which was also confirmed by the Youden index values being far from one. In conclusion, the assessment of milk bovine neutrophil functions could improve our understanding of the cellular basis of mastitis. Although, the intracellular ROS production and S. aureus phagocytosis by milk neutrophil did not have high predictive values to detect intramammary infections, our results strengthen the idea that that poor bovine mammary gland neutrophil phagocytic ability may be associated with high SCC, and might be considered to identify susceptible dairy cows to mastitis.

A resposta imune da glândula mamária desempenha um papel importante ao determinar o estabelecimento da dos neutrófilos irá nos subsidiar conhecimentos, pelo qual podemos evitar o processo inflamatório pela otimização da resposta imune de bovinos leiteiros, e fornece ferramentas para diagnosticar a mastite ou um possível instrumento para identificar e selecionar animais resistentes à infecção intramamária, aumentando a produtividade do rebanho. O presente estudo utilizou 112 amostras provenientes de quartos mamários de 28 vacas Holandesas que foram divididos em amostras de leite com baixa (n=72; <2×105 células mL-1) ou alta (n=40; 2×105 células mL-1) contagem de células somáticas (CCS). A porcentagem de neutrófilos no leite, a produção intracelular de espécies reativas de oxigênio (ERO) e a fagocitose de Staphylococcus aureus pelos neutrófilos do leite foram avaliadas por citometria de fluxo. Os resultados do presente estudo demonstraram maior percentagem de neutrófilos (CH138+; P=0,0003) e percentagem de neutrófilos que produziram ERO (P=0,008) em amostras de leite com alta CCS. Por outro lado, a intensidade de fagocitose de S. aureus por neutrófilos em amostras de leite com baixa CCS (P=0,003), que demonstra maior atividade funcional destas células neste grupo. As funções neutrofílicas para o diagnóstico da mastite não apresentaram valores de sensibilidade e especificidade altos, que foram confirmados pelo índice Youden. Desta forma, conclui-se que a produção intracelular de ERO e fagocitose de S. aureus pelos neutrófilos do leite não apresentaram valores preditivos altos para detecção de mastite. Além disto, os resultados do presente estudo reforçam a ideia de neutrófilos do leite com menor capacidade fagocítica podem ser associados à alta CCS, e pode ser considerado como uma ferramenta para identificar animais mais susceptíveis à infecções intramamárias.

E-mail: arinepel@yahoo.com.br Hypertrophic cardiomyopathy (HCM) is the most common feline heart disease and is characterized by increased cardiac mass with a hypertrophied and not dilated left ventricle. The echocardiography is the best noninvasive diagnostic tool for the differentiation of cardiomyopathies and is considered the gold standard for detection of ventricular hypertrophy present in HCM. Electrocardiographic changes are also common in animals with HCM and the electrocardiogram (ECG) is quick, easy and highly available screening test for the detection of ventricular hypertrophy in humans. In cats, few studies have been conducted regarding the sensitivity and specificity of ECG in detecting ventricular hypertrophy. With the intention of evaluating the use of ECG as a screening tool for diagnosis of HCM in cats, Persian cats (n=82) were evaluated by echocardiographic and electrocardiographic examinations. Animals with blocks and/or conduction disturbances were excluded from statistical analysis (n=22). Subsequently the animals included were classified as normal (n=38), suspicious (n=6) and affected by HCM (n=16). Statistical differences were observed in the P-wave amplitude in DII and R-wave amplitude in DII, CV6LL and CV6LU, with higher values in animals with HCM. Velocities and pressure gradient of aortic flow, left atrial diameter (LA) and LA/Ao ratio were higher in cats with HCM. Among the animals with ECG changes suggestive of left atrial enlargement (n=7), only two actually had LA enlargement on echocardiography, and among animals with left atrial enlargement on echocardiogram (n=7), only two had ECG changes suggestive of overload AE (40,4% of sensibility and 90,9% of specificity). Among the animals with ECG changes suggestive of left ventricular hypertrophy (n=6), five actually had ventricular hypertrophy on echocardiography, and among animals with HCM by echocardiography (n=16), only five showed electrocardiographic abnormalities suggestive of LV hypertrophy (31,25% of sensibility and 97,72% of specificity). We observed a positive correlation between diastolic thickness of the interventricular septum and/or left ventricular free wall and R-wave amplitude in DII and CV6LU. The electrocardiogram is quick and easy to perform, has good specificity in detecting ventricular hypertrophy in cats, however, has low sensitivity, with large numbers of false negative animals. Thus, the ECG assists in the diagnosis, but does not replace echocardiography in confirming ventricular hypertrophy.

O ecocardiograma é o melhor meio diagnóstico não invasivo para a diferenciação das cardiomiopatias e é considerado padrão ouro para a detecção de hipertrofia ventricular presente na CMH. Alterações eletrocardiográficas também são comuns em animais com CMH e o eletrocardiograma (ECG) é um teste de triagem para detecção de hipertrofia ventricular em humanos, sendo um exame rápido e facilmente disponível. Em gatos, poucos estudos foram realizados quanto à sensibilidade e especificidade do ECG na detecção de hipertrofia ventricular. Com a intenção de avaliar o uso do ECG como ferramenta de triagem para diagnóstico de CMH em felinos, gatos da raça Persa (n=82) foram avaliados por meio de exames ecocardiográfico e eletrocardiográfico. Animais com bloqueios e/ou distúrbios de condução foram excluídos da análise estatística (n=22). Posteriormente, os animais incluídos foram classificados em: normais (n=38), suspeitos (n=6) e acometidos pela CMH (n=16). Observaram-se diferenças estatísticas na amplitude da onda P em DII e na amplitude de onda R em DII, CV6LL e CV6LU, com valores maiores nos animais com CMH; e nos valores ecocardiográficos de velocidade e gradiente de pressão do fluxo aórtico, diâmetro do átrio esquerdo (AE) e relação AE/Ao, com valores maiores nos gatos com CMH. Dentre os animais com alterações eletrocardiográficas sugestivas de sobrecarga atrial esquerda (n=7), apenas dois realmente apresentavam aumento do AE no ecocardiograma; e dentre os animais com aumento atrial esquerdo ao ecocardiograma (n=7), apenas dois apresentavam alterações eletrocardiográficas sugestivas de sobrecarga do AE (sensibilidade de 40,40% e especificidade de 90,90%). Dentre os gatos com alterações eletrocardiográficas sugestivas de sobrecarga ventricular esquerda (n=6), cinco realmente apresentavam hipertrofia ventricular ao ecocardiograma; e dentre os animais com CMH ao ecocardiograma (n=16), apenas cinco apresentaram alterações eletrocardiográficas sugestivas de sobrecarga do VE (sensibilidade de 31,25% e especificidade de 97,72%). Observou-se correlação positiva entre espessura diastólica do septo interventricular e/ou da parede livre do ventrículo esquerdo e a amplitude da onda R em derivações DII e CV6LU. O eletrocardiograma é um exame rápido e de fácil execução, apresenta boa especificidade na detecção de hipertrofia ventricular em felinos, porém, possui baixa sensibilidade, com grande número de falsos negativos. Desta forma, o ECG auxilia no diagnóstico, mas não substitui o ecocardiograma na confirmação da hipertrofia ventricular.

The toxicological sensitivity and specificity of the micro-complement fixation test (MCF) for the detection of botulinum toxins C and D were studied in supernatants of the bacterial cultures and in livers of mice inoculated with lethal and sublethal doses. Botulinum toxins C and D were produced in Hemoline culture medium, titered through the determination of LD50 by the mouse test and adjusted to dilutions of 10, 1, 0.1, 0.01 and 0.001 LD50 Two experimental models were used to determine the toxicological sensitivity of MCF in the supernatant of the culture medium with the dilutions described, and also in liver extracts of mice weighing 20 g and inoculated with the same dilutions. Detection of the botulinum toxins was attempted in liver extracts of mice which had received lethal doses of the respective toxins, and in others which had been inoculated with sublethal doses and were sacrificed in intervals of 5 days. The results show that the toxicological sensitivity of MCF, regarding the two types of toxins at the level of 0.001 LD50, was 100% when the supernatants of the culture medium were tested; this means that the sensitivitywas 100 times higher than with the mouse test. The toxicological sensitivity of MCF in the liver extracts of mice inoculated with 1 and 10 LD50 of botulinum toxins C and D was inferior, giving values of 100, 80, 89 and 72% respectively. By this test it was also possible to detect botulinum toxins type C and D in liver extracts of mice inoculated with sublethal doses, up to 15 days after the injection. The specificity of MCF was 88% and 92%, when liver extracts of healthy contral mice were tested and when challenged with antitoxins C and D; and 100% when challenged with the supernatant of the culture medium. These results indicate that MCF could be of importance for research and could substitute in vivo tests.

No presente estudo pretendeu-se verificar a sensibilidade toxicológica e especificidade do Teste de Microfixação de Complemento (MCF) na detecção de toxinas botulínicas C e D no sobrenadante de cultivas bacterianos e em fígados de camundongos inoculados com doses letais e subletais. As toxinas foram produzidas em meio de cultura Hemoline, tituladas através da determinação da DL50 pelo Bioensaio em Camundongo e diluídas nas concentrações de 10, 1, 0, 1, 0,01 e 0,001 DL50. Desta forma, foram utilizadas em dois modelos experimentais, onde foi determinada a sensibilidade toxicológica do MCF no sobrenadante do meio de cultura com as diluições descritas acima e ainda em extratos hepáticos de camundongos com peso corporal de 20g, inoculados com as mesmas diluições. A tentativa de evidenciação das toxinas botulínicas nos extratos hepáticos de camundongos foi realizada através da sua extração após a morte pela administração das doses letais e ainda pelo sacrifício dos animais inoculados -com doses subletais, em intervalos de 5 dias. Os resultados evidenciaram uma sensibilidade toxicológica para o MCF de 100% para os dois tipos de toxinas ao nível de 0,01 DL50, quando testados os sobrenadantes de meio de cultura, portanto 100 vezes superior ao Bioensaio em Camundongo. A sensibilidade toxicológica do MCF, quando examinados extratos hepáticos de camundongos inoculados com 1 e 10 DL50 de toxinas botulínicas C e D, foi inferior, com valores de 100, 80, 89 e 72%, respectivamente. Pelo teste foi possível detectar toxinas botulínicas tipos C e D nos extratos hepáticos de camundongos inoculados com doses subletais até 15 dias após a sua inoculação. A especificidade do MCF foi de 88 e 92%, quando testados extratos hepáticos de camundongos sadios, e confrontados com as antitoxinas C e D; e 100% no sobrenadante do meio de cultura. Os resultados apontam para uma possível utilização do teste como importante instrumento de pesquisa e ainda na eventual substituição dos testes in vivo pelas suas implicações éticas e limitações práticas.


#4 - Sensitivity and specificity of the Califomia Mastitis Test as a diagnostic tool for subclinical mastitis in quarter somatic cell count estimation

Abstract in English:

The California Mastitis Test (CMT) is a simple and widely used diagnostic tool for subclinical mastitis. lt is used even in areas where laboratory facilities are available for diagnosis and monitoring program purposes. CMT usually score 1-5, where 1 indicates a completely negative reaction and 2-5 increasing degrees of inflammatory reaction of the udder. The reactions 2-5, 3-5 or 4-5 may be considered as indicative of subclinical mastitis, because they are related to an increase of the somatic cell count (SCC) in the milk. This variation, however, may produce either false-positive or false-negative results. The aim of this study was to assess the sensitivity and specificity of the CMT in comparison with SCC. A total of 3,012 quarter milk samples from 760 lactating cows were examined. CMT was evaluated at the moment of sampling at cow side and SCC in the laboratory, with a fluoro-opto-electronic method (Fossomatic 90). The average CCS (x 1,000 cells/ml) for CMT scores were: 1 (79.9), 2 (333.5), 3 (670.3), 4 (1,354.0) and 5 (4,455.6). Three options for CMT interpretation were evaluated in relation to a range of CCS, starting from 100,000 cells/ml: (a) 1 versus 2, 3, 4, and 5; (b) 1 and 2 versus 3, 4 and 5; and (e) 1, 2 and 3 versus 4 and 5. The sensitivities of CMT scoring to detect quarters with SCC >200,000/ml were 79%, 61% e 34%for options a, b ande, respectively. The sensitivities of CMT scores for SCC >500,000/ml for options a, b ande, were, respectively, 93%, 82% e 54%. The sensitivity of CMT scores to identify subclinical mastitis was satisfactory (above 80%) when options b ande were used. The interpretation of CMT score 3 as negative for subclinical mastitis was considered as adequate (sensitivity around 80%) only when CCS ranged between 1,200,000 and 1,400,000 cells/ml. The specificities of CMT scoring for CCS of 200,000 and 500,000 were, respectively, 90% and 80% (option a), 97% and 90% (option b) and 99% and 97% (option e).

O "California Mastitis Test" (CMT) estima o conteúdo de células somáticas no leite e é interpretado subjetivamente, estabelecendo-se escores que, na maioria dos casos, variam de 1 a 5. O escore 1 indica uma reação completamente negativa e os de 2-5 indicam graus crescentes de resposta inflamatória do úbere, sendo normalmente considerados como indicativos de mastite subclínica. Dependendo da interpretação dos escores, o CMT pode produzir resultados falso-positivos ou falso-negativos. Esse trabalho teve o objetivo de avaliar a sensibilidade e a especificidade do CMT em relação à contagem de células somáticas (CCS). Foram utilizadas 3.012 amostras de leite provenientes de 760 vacas Holandesas ou mestiças Holandês-Zebu, pertencentes a 15 rebanhos. Todas as amostras foram submetidas ao CMT e processadas para CCS em equipamento Fossomatic 90. Os valores médios de CCS (x 1.000 células/mi) obtidos para os escores de CMT foram 1 (79,9), 2 (333,5), 3 (670,3), 4 (1.354,0) e 5 (4.455,6). Três opções de interpretação (doente/não-doente) para o CMT foram testadas, em relação aos valores de CCS, iniciando com 100.000 células/ml: (a) 1 versus 2, 3, 4, e 5; (b) 1 e 2 versus 3, 4 e 5; (c) 1, 2, 3 versus 4 e 5. As sensibilidades do CMT em identificar corretamente quartos mamários acima de 200.000 células/mi foram 79%, 61% e 34%, para as opções a, b e e, respectivamente. Para identificar corretamente contagens acima de 500.000 células/mi, as sensibilidades do CMT, para as opções a, b e e, foram, respectivamente: 93%, 82% e 54%. A sensibilidade do CMT em identificar quartos mamários com mastite subclínica foi adequada (acima de 80%) quando a interpretação do teste foi mais rigorosa (opções a e b). A interpretação da reação 3 como negativa (opção e) só alcançou sensibilidade de 80% para contagens entre 1.200.000 e 1.400.000 células/mi. As especificidades do CMT, para CCS de 200.000 e 500.000 foram, respectivamente, 90% e 80% (opção a), 97% e 90% (opção b) e 99% e 97% (opção e).
